If you are bringing in a kitty that has been to Pretty Kitty Cat Grooming before, you may use the form below. Otherwise, for kitties that have not been to Pretty Kitty Cat Grooming in the past or if your contact information has changed, please use the New Client Release.
Vaccinations
* Vaccinations are not currently required but are strongly recommended. If not vaccinated, you release Pretty Kitty Cat Grooming from all liability if your cat becomes ill.
Emergencies
In the event of an emergency, I authorize this establishment to immediately seek professional veterinary attention for my pet (at my expense). I understand that all attempts will be made to contact me in the event of an emergency.
Coat Condition
I understand that this establishment puts my pet’s comfort above all else. In the event that my cat’s coat is severely matted, I understand that the groomer may have to shave my cat rather than perform a painful de-matting procedure. I also understand that if my cat is severely matted, that there is an increased risk of clipper burn or nicks to occur. I understand that all attempts will be made to prevent this, however in many extreme mat conditions (such as pelting), it can be unavoidable. I also understand that matted cats take additional time to groom and cause greater wear and tear on equipment, so there will be an additional fee added on to the regular grooming price if my cat’s coat is matted.
Health
I understand that grooming can be stressful to some cats and I will inform the groomer if my cat has any known heart conditions or any stress-aggravated issues prior to grooming. I also understand that if the groomer determines that my cat is too stressed, the grooming session will be halted and the grooming plan will need to be adjusted or completed at a later date. I understand that I will be contacted in such an event. I understand that current rabies and FVRCP vaccines are strongly recommended for the safety of all clients’ cats. Further, I understand that the establishment is not responsible for any vaccine-preventable infections if I disregard this recommendation. For the health and safety of all Pretty Kitty Cat Grooming clients, I acknowledge that my cat is not exhibiting any unusual respiratory symptoms and if such symptoms develop prior to my appointment, I will cancel or reschedule my appointment.
Cancellation Policy
I understand that if I need to change or cancel my appointment, that I must give at least 24 hours notice so that the appointment time can be made available to another client. If two appointments are missed without 24 hours notice, I may be required to pre-pay prior to scheduling any future appointments.
COVID-19 Policy
I hereby state that to the best of my knowledge I have no symptoms that could be attributed to COVID-19 including, but not limited to fever, cough, difficulty breathing, or common cold symptoms, and that no one in my household exhibits these symptoms at this time. I agree that I am not knowingly bringing COVID-19 into the Pretty Kitty Cat Grooming facility. If I become ill before my appointment I will cancel or reschedule my appointment.
